a) Application form; b) Copy of national passport valid at least 1 year from entry date into Moldova; c) Copies of education documents (high-school diploma or equivalent), translated into Romanian/English/Russian, notarized and legalized (Apostille for Hague Convention states; otherwise Foreign Ministry stamp + legalization at General Consular Directorate of Moldova) — NEET certificate for candidates from India; d) Copy of language proficiency certificate; e) Notarized copy of birth certificate; f) Notarized parental consent for applicants under 18; g) Copy of marriage certificate (if applicable); h) Declaration of responsibility for veridity and legality of submitted documents; i) 1 digital photo 3x4 cm + 2 matte photos 3x4 cm. Documents sent as 300 dpi scans in ONE email to foreign@usmf.md, each document in a separate file named Student_Name_Document_Name.
